The origins of the church building date back to a convent founded in the late 9th century and named after Saint Cecilia of Rome. It was long assumed that the first Cologne Cathedral stood on this site. However, excavations have ruled out this tradition. Instead, the foundations of a rectangular church were found, which probably belonged to the convent. According to documents relating to the convent's difficult economic situation in the 10th century, Archbishop Brun bequeathed 50 pounds of silver to the convent in 965 for the completion of the church. It appears that the crypt, rebuilt in 1970 to the west of the church, was first constructed around this time to house relics. The Archbishop of Cologne used to celebrate the second Christmas Mass in St. Cecilia's Church after St. Mary in the Capitol, thus emphasizing the special significance of the convent, which declined over the course of the Middle Ages.

In 1869, the church council of Sand decided to build a new church because the old church in the Old Churchyard in Sand had become too small and in need of renovation. During construction, the church tower collapsed on November 21, 1879, before it had even received a proper roof. However, construction progressed quickly, and the church was consecrated by Dean Causemann of Bensberg on October 23, 1881. The bishop consecrated the church on July 28, 1886. Initially, the 1851 organ from the old church was installed. However, as this had suffered greatly during transport, it was replaced with a new organ in 1908. The church bells had to be handed over during both world wars. They were replaced by three new bells in 1924 and five in 1957. The church received a new organ in 1989.
