It is possible that originally there was a Basilian cave, on which a chapel was later built which, if linked to the church of the Holy Trinity, would seem to be mentioned in the report of the pastoral visit of the bishop of Mileto Monsignor Marcantonio Del Tufo, which took place in 1586.

Having fallen into disuse, the Marquis Giovanni Domenico Milano financed its restoration; it was dedicated to Sant'Anna with the inauguration ceremony in 1728. [4] When in 1783 Polistena was hit by a terrible earthquake, the church resisted. [5] The country was however destroyed and there were 2,000 dead. The religious activity in the church was reduced for many years to the sole celebration of the feast of the saint, in July. In the archive of the Archconfraternity of the Holy Trinity of Polistena a ceremony of new blessing of the church is recorded, and consequent reopening, which took place on December 24, 1810. [6]

The church, like other religious buildings in Polistena, was used as a burial place until 1877.

In the two-year period 1908-1909, the church was again the subject of restoration interventions, financed by the citrus merchant Ignazio Martino, and based on designs by prof. Francesco Morani the marble altar was built in 1962 as a sign of devotion to the saint. The Martino family then continued their devotional work and took on urgent consolidation work in 1990.
