The historical development of the Hospital Church is connected to the origins of the formerly adjacent hospital and cemetery.
At the end of the 13th century, a leper house (retirement home) was built outside the then city (now the Old Town) on what is now the Marktgraben. A cemetery was added in the 14th century. With the closure of the parish cemetery near St. James's Church (now St. James's Cathedral) in 1509, the hospital cemetery was enlarged, and a Gothic St. Vitus Chapel was built on its west side. This cemetery was relocated to the West Cemetery in 1856. At the same time, the Gothic cemetery chapel was demolished. In 1869, an extension to the hospital was built there, but the hospital was relocated to what was then the western edge of the city, today's hospital grounds, in 1888. In 1890, the old hospital building was converted into a school.
A chapel dedicated to the Virgin Mary may have been connected to the leper house "in the New Town" from the very beginning. The associated patronage of Mary has always been anchored in the street-side (left) nave. In connection with the founding of the Holy Spirit Hospital, a new nave dedicated to the Holy Spirit was added to the west.
In 1389, the term "ecclesia" was first mentioned – presumably because the two-aisled chapel had been expanded to include a choir and tower.
During the reconstruction and expansion of the hospital church by master mason Christoph Geiger and master carpenter Adam Rausch in 1596, a three-aisled church was created by incorporating the St. Elsbeth Chapel.
In 1689, the church was severely damaged by an earthquake. In 1700, it was demolished except for a few outer walls and foundations. The previous building was square. This bothered the building commission, so the church was extended by the addition of the gallery bay. It was built by the Innsbruck court architect Johann Martin Gumpp and consecrated on June 8, 1705, by the Prince-Bishop of Brixen, Kaspar Ignaz Count Künigl. Some of the furnishings were added later.
